1/5/2024 EnchantaVaughn Approved (AM) Upon arrival, client hugged QMHP on her way to her assigned seat for the WInter Program. Client was observed displaying appropriate manners in class and listening to morning announcements. Client appeared to be in a positive mood, as evidenced by her happy spirit and positive attitude. Client was observed following directions and giving counselors her undivided attention. Client was observed studying and preparing for her test. Client seemed to be in a jovial mood, as evidenced by her smile and positive temper. Client stated she was going to try her best and take her time today. QMHP expressed how proud she was of her doing well in class, as well as, transitioning and want her to keep up the good work and behavior. Client expressed good manners, at this time and was in a cheerful mood going into lunch. (PM) Client was respectful and polite to staff while showing good manners. Client picked up her food and sat down and seemed to be happy and content with being around her friends. Client displayed good social behaviors in the cafeteria. Client was respectful and polite to cafeteria staff. Client displayed normal activity before transitioning to see her counselor and begin group session. Client was encouraged to get comfortable and prepare for a group activity. Client avoided inappropriate behaviors throughout session and was praised for being on task. Client was educated about school rules and regulations and why it’s important to follow rules. Client showed understanding by nodding and smiling. Client demonstrated excellent interaction by communicating her feelings and sharing her experience. Client exhibited a positive attitude and did a great job reflecting and engaging in the listening activity. Client asked appropriate questions and gave appropriate feedback. Client was able to distress and calm down without medication and she was rewarded for that accomplishment. (EOD) Client was seen in class following directives and contributing to group discussion. Client was observed paying attention to teacher while teaching lesson and was able to respond appropriately to discussion. Client was observed displaying good behaviors at her desk, utilizing proper voice volume and pleasant interaction with her classmates. Client completed and turned in her work on time. Client seemed to be coping fine with stressors and utilizing positive reinforcers given by authority figures. Client did not require to be redirected by teachers or TDT staff. Client began preparing for dismissal by packing up and organizing her belongings and clearing off her desk. Client displayed good listening skills when announcements were on and left school is a good mood. Client dismissed safely and was able to obtain ISP objectives.
